Does Claude Code's abilities fluctuate for you too?

In the project I've been working on in the past weeks with Claude Code, I have very clear instructions in the CLAUDE.md file. Things like:- I'll push my work on branch X on this repo. Always fetch X to see if there is something new to merge- Never touch files Y. These must be edited by hand, by me.- If you make any changes to file Z, you must always explicitly ask for my confirmation.It used to work very reliably. But for the past two days, Claude Code suddenly failed to follow these instructions several times, in different sessions, and right now even after the first prompt in a new session. Early on in my professional life, I worked in a psychiatric secure ward, meaning that the patients could not leave.
I seemed to get drawn in to the world of my patients, luckily I had constant clinical supervision.
one patient was well known for telling huge lies about his life. He told me had had been a professional drummer in a well known band and that they had a few famous records that could be heard of Youtube.
I was naive and still learning my trade craft. I looked up the bands name and searched youtube.
I was really hurt when my supervisor told me this story was untrue.
She said "you seem to forget that all the people in here suffer from extreme forms of mental illness.
So I say to you:
you seem to forget that you are dealing with a machine and not a human.
