Do you accidentally flag submissions?

After a story disappeared when I tried to click it (on mobile), I checked flagged submissions under my profile.

To my dismay, there were dozens of submissions on that list, nearly all of which seemed like perfectly fine submissions.

I often downvote comments by accident, but thankfully that's easy to undo.

Does this happen to you, and what do you do to avoid it?
