How do you use pen and paper to document off-the-cuff thoughts/ideas?

Ideas, thoughts occur at anytime. And inter-related thoughts/ideas may occur at different times. In that case, how does one effectively document these using pen and paper?

Do you maintain a notebook? Will it not fill-up quickly, and won't it be difficult to link ideas/thoughts that are not written on the same page? And do you carry your notebook always with you wherever you go?

I consider the most effective part of using pen and paper for developing my another great-but-vague-formulated-idea is that if I write something down then in few minutes I am having a lot of different facts on my sheet. I look at the sheet and produce a new extension of my idea which might be way harder without seeing that much of disturbing me facts at once.

And I never reread my notebooks full of notes, because after some extension has developed, all previous writings use to become stupid-looking and outdated because they do not have the extension.

I'm the kind of guy that has 1-2 notebooks in my backpack at all times.

Bullet journaling system worked for me when I wanted to organize my notes. Numbered pages and a table of contents help in navigation.
