Getting therapy, particularly talking therapy with a qualified therapist. I know this doesn't answer your question exactly, but it'll very likely help you uncover the root of the anxiety and manage it way better. It's one of the best things I can recommend. If you must go down the self help route, I'd consider looking at physical changes as well as the things your looking for, such as exercise, diet, relationships, getting outdoors, and so on. Get yourself diagnosed properly. I let my anxiety control my life for far too long, now I am on the correct drugs, my quality of life is WAY better.
