Is peer-to-peer connections possible on the Internet?

Assuming you are not behind CGNAT, yes it has always been possible. One or more people will have to allow ports to be forwarded to their internal private rfc1918 network. Carrier Grade Nat (I don't like this term) breaks this model and you would need a STUN/TURN server or equivalent to manage and route the connections.
