Yes, accidentally forgetting to change your env from prod when running an SQL query that impacts a lot of rows.
As in when working on a software project that involves firearms and you're checking the weapons on the table to be completely freaked out by the people present in the room who are supoosed to have had weapons training who pick up weapons from the table, look at the muzzle, and actually pull the fucking trigger, and having someone almost sweep you with a shotgun but your spidey senses make you involuntarily seize the weapon and force it to be put on the table, and then go to the armorer and tell him "Who exactly will handle these, and can we make sure only required people are here?"
That kind of hypothetical scenarios?
I've never heard of firearms being used in an IT workplace. I guess it's possible that somebody working in IT could carry a gun, but I can't imagine how it would become a necessary part of their job to use it.
Mishaps involving test/production environments happen a lot
How about either code or config changes to gear that runs in hazardous environments such as factories, mills, gas/electricity/fuel environments etc. power plants.. the list goes on.
One in which a person dies? I can't think of anything. Maybe people have been crushed by data center cages in the process of being attached to the wall, but not quite yet?
